Before you file
- Most prescriptions are billed at the pharmacy through the PBM. You may file for reimbursement when you paid cash at a retail or mail-order pharmacy outside normal routing, subject to plan rules.
What to gather

Pharmacy receipt with drug name, strength, quantity, date filled, NDC, and amount you paid.

How to submit (you send it)

Use the member portal or paper/fax path your employer’s Express Scripts materials describe; form names change by account.
